Can you explain why there are so many errors in medical literature? This is a problem that has bothered me a long time.

Sometimes conclusions contradict results, or numerical results are clearly impossible based on described methodology. Sometimes basic anatomy is described incorrectly or mislabeled.

Typically when providing references for claims, authors do not cite primary sources, and what ensues is like a game of telephone.

Why is peer review not working to ensure better information quality?
