There is actually a great deal done to fight this and it is illegal. I am not aware the penalties for this are any less than for rape. Are they?

I personally had my clitoris denervated in a surgery performed without my consent by a male OB/GYN, and he went on to become president of the Texas Medical Association.

Intention does matter. If you do something to another person, without their consent, for your own sexual gratification, that is considered worse than doing something according to a custom, by accident, or because you believe you are helping the other person.
